New Patients

New patients may complete their patient forms prior to their appointment. If forms have not been completed in advance, please arrive 15 minutes before your scheduled appointment time to allow for completion.

At your initial consultation, your podiatric surgeon will take a detailed medical history and perform an examination of your foot and ankle to assist in establishing a diagnosis. Additional investigations may be required where clinically indicated.

Our reception team will provide an online form at the time of booking. Alternatively, a form can be downloaded prior to your appointment.

Preparing for Your First Consultation

To assist with assessment, please bring the following items to your appointment:

  • Referral letter

  • Previous operation reports if applicable or relevant medical correspondence

  • Relevant X-rays, scans, and imaging reports

  • Recent blood tests or investigation results

  • Workers’ compensation or CTP details (if applicable), including:

    • Insurer name

    • Claim number

    • Case manager contact details

Please wear loose-fitting clothing (such as shorts or trousers) to allow appropriate examination of the lower limb.

Providing relevant clinical information at your initial consultation assists in assessment and management planning.

Consultation Fees

Consultation fees apply and will be advised at the time of booking.

Payment is required on the day of consultation. Medicare and private health fund rebates (where applicable) may be processed following payment.
